community. Required Skills: Current and active Iowa Medical License
Certification as a Nurse Practitioner (APRN) Proficiency in
performing health assessments and risk evaluations Strong
communication and organizational skills Comfort with seeing
pediatric and adult patients Ability to operate telehealth
platforms effectively Enrolled and credentialed in Medicard
Reliable transportation for in-home visits Nice to Have Skills:
Experience in home health or telehealth settings Knowledge of
chronic disease management Bilingual abilities to serve diverse
populations Familiarity with electronic health records and
Salesforce Preferred Education and Experience: Master’s degree in
Nursing or related field At least 1-2 years of clinical practice as
a Nurse Practitioner Prior experience in community or home health
preferred Other Requirements: Ability to travel within assigned
counties; mileage reimbursed at IRS standard rate Availability for
a daytime shift, Monday through Friday, 8 am to 5 pm Must
successfully complete 1-3 hours of paid orientation; ongoing
education to be completed during work hours Commencement of
employment starting 03/09/2026 This role offers a competitive
